1. Initial Information Gathering & Preparation (Before Your Appointment)

 

To ensure the most thorough and efficient evaluation, we request relevant information in advance, which can be securely uploaded via our patient portal:

  • Intake Questionnaires: Complete detailed forms about your child’s developmental milestones, medical history, social experiences, and current concerns across all key domains (communication, motor skills, social-emotional, adaptive skills).
  • Previous Reports: Submit any prior evaluations from psychologists, speech therapists, occupational therapists, or school records (IFSP, IEP, ETR, academic reports). This helps Dr. Nallapati avoid redundant testing and focus on new or missed insights.
  • Teacher/Caregiver Input: We welcome observations from educators or caregivers about your child’s behavior and development in different settings.

 

2. The Initial Telehealth Evaluation Appointment

 

The core evaluation is conducted entirely remotely, providing a comfortable and familiar environment for your child.

  • Note on Physical Exam: As a fully remote practice, a physical and neurological exam is not conducted. However, if history or virtual observations raise concerns, we will guide you on obtaining an in-person evaluation with your pediatrician or provide referrals to specialists like a geneticist or neurologist if deemed necessary.
